Silver Creek Physical Therapy, a member of Therapy Partners Group, is hiring an Outpatient Physical Therapist Assistant to join a collaborative team serving San Jose and surrounding communities. The role includes implementing therapies under a PT's guidance and contributing to patient care plans.

The ideal candidate holds an accredited PTA degree, current licensure or eligibility, and a commitment to excellent patient care. Outpatient setting; competitive salary and benefits offered.

Qualifications

• Associate degree in Physical Therapy Assistant from an accredited program.
• Current state licensure or eligibility for licensure as a PTA.
• Outpatient experience preferred but not required.

Responsibilities

• Follow up with patients; conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Assist physical therapists in developing and implementing patient treatment plans.
• Implement therapeutic techniques including manual therapy, exercise prescriptions, modalities, and patient education.
• Document patient progress using Raintree and report changes to the physical therapist.
• Educate patients and families on exercises, pain management techniques, and care plans.
• Collaborate with the healthcare team to ensure optimal patient care and safety.

Key Responsibilities

• Patient Follow Up: Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Treatment Planning: Assist physical therapists in developing and implementing patient treatment plans.
• Therapeutic Interventions: Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: While using Raintree, Monitor and document patient progress during treatment sessions, reporting changes to the physical therapist.
• Patient Education: Educate patients and families on exercises, pain management techniques, and care plans
• Professional Atmosphere: Work collaboratively with the healthcare team to ensure optimal patient care and safety.

Qualifications

• Associate Degree as Physical Therapy Assistant from accredited institution.
• Current state licensure or eligibility for licensure as a Physical Therapist Assistant.
• Previous outpatient experience preferred but not required.
• Ability to work collaboratively in a team-oriented environment.
