Silver Creek Physical Therapy is seeking an Outpatient Physical Therapy Assistant to join a dedicated, collaborative team supporting orthopedic care in the San Jose area. The role focuses on executing prescribed exercises, modalities, and education under supervision, ensuring high-quality patient outcomes.

The candidate must have an PTA degree, be licensed or eligible to practice in the state, and thrive in a team-focused environment with opportunities for professional growth and advancement.

Qualifications

• Associate Degree as Physical Therapy Assistant from accredited institution.
• Current state licensure or eligibility for licensure as a Physical Therapist Assistant.
• Previous outpatient experience preferred but not required.
• Ability to work collaboratively in a team-oriented environment.
• Commitment to providing exceptional patient care

Responsibilities

• Patient Follow Up: Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Treatment Planning: Assist physical therapists in developing and implementing patient treatment plans.
• Therapeutic Interventions: Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: While using Raintree, Monitor and document patient progress during treatment sessions, reporting changes to the physical therapist.
• Patient Education: Educate patients and families on exercises, pain management techniques, and care plans
• Professional Atmosphere: Work collaboratively with the healthcare team to ensure optimal patient care and safety
