Job summary

Apex Home Health, Hospice & Home Care seeks a qualified Physical Therapist Assistant to provide physical therapy interventions under the supervision of a licensed Physical Therapist (PT). You will work in patients' homes and care facilities, following PT care plans, monitoring progress, documenting care, and adapting treatments within the scope of practice.

The role requires NY State PTA certification, at least two years of clinical experience, and a valid driver’s license may be required.

Qualifications

• New York State Physical Therapist Assistant certification.
• Minimum of two years clinical experience.
• Possession of a valid driver’s license may be required.
• Associate’s Degree from an approved program; bilingual skills and basic PC skills preferred.
