Job summary

Crossroads Home Health and Hospice is seeking a Physical Therapy Assistant in California. The PTA will provide PT services under the direction of a licensed PT and according to an individualized care plan.

The position requires an accredited PTA program graduate, current state licensure, and CPR certification; valid driving credentials preferred for home visits. The role includes patient education, documentation, and participation in an interdisciplinary team.

Qualifications

Responsibilities

• Provide direct physical therapy under the direction of a licensed physical therapist and according to the plan of care.
• Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
• Direct and aid patients in therapeutic exercises, gait training, transfers, and prosthetic training.
• Utilize therapeutic modalities (heat, cold, water, light, electricity, ultrasound, massage) as directed.
• Observe and report patient responses to treatment and any changes in condition to the supervising therapist.
• Instruct patients in wheelchair, brace, crutch, cane, and prosthetic/orthotic device usage.
• Maintain clinical records and notes within prescribed timeframes.
• Participate in interdisciplinary team and Inservice Education programs.
• Contribute to quality improvement activities.
• Performs other duties as assigned

Job Description Summary

The physical therapy assistant, contract or employed, provides physical therapy services under the direction of the physical therapist and according to the individualized plan of care.

Essential Job Functions/Responsibilities

• Provides direct physical therapy according to directions of a qualified physical therapist and in accordance with the plan of care and within the scope of the Physical Therapy Practice Act.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
• Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
• Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.
• Participates in interdisciplinary group and Organization Inservice Education Program.
• Participates in the quality assessment performance improvement teams and activities.
• Performs other duties as assigned.

Position Qualifications

• Graduate of an accredited physical therapy assistant program with either an Associate’s degree in science or applied science that is approved by American Physical Therapy Association, Inc. Meets personnel qualifications stated in 418.114(b)(8).
• Current certification or license as required by state law.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
