Position Summary

Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

As a Physical Therapist Assistant, You Will

• Perform and document initial exams, evaluating data to identify problems and determine diagnoses prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Apply concepts of infection control/universal precautions in patient care.
• Ensure continuity of quality patient care with proper documentation.
• Provide effective communication to patients, families, team members, and other healthcare professionals.
• Promote agency philosophy and administrative policies.
• Maintain and complete all documentation within company guidelines.

Requirements

• Successful completion of an accredited physical therapy program and passing the state licensure exam.
• Valid Physical Therapy license.
• Valid state driver’s license.
• Preferred: 1 year of home health experience.
• AHA CPR certification.
• Valid auto insurance.
• Experience with HomeCare HomeBase is a plus.

Physical Demands

• Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Ability to lift, carry, pull, or push up to 100 lbs.
