Physical Therapist Assistant (PTA) - Northeast, Woodlands, and Southeast

Join us at Transition Home Healthcare as a Physical Therapist Assistant (PTA) serving Houston, TX, and surrounding areas since 2010. We are committed to providing high-quality home healthcare services and are looking for dedicated professionals to join our team.

Position Details
  • Shifts Available: Full-Time & PRN
  • Service Areas: Northeast, Woodlands, and Southeast
Responsibilities
  1. Assist in the rehabilitation of patients with emotional, physical, or mental disabilities.
  2. Implement physical therapy care plans.
  3. Coordinate care with the home healthcare team.
  4. Educate patients and families about health conditions.
  5. Prepare accurate clinical notes in a timely manner.
  6. Communicate effectively with patients, caregivers, and healthcare team members.
Qualifications
  • Licensed Assistant Physical Therapist in Texas.
  • Home health experience preferred.
  • Knowledge of home health regulations and healthcare compliance.
  • Experience with HCHB EMR (preferred but not required).
  • Ability to handle physical workload.
  • Good driving record.
  • Excellent communication and interpersonal skills.
