Boost Home Healthcare is seeking a licensed Physical Therapy Assistant to deliver physician-prescribed physical therapy in patients’ homes in Fort Collins, Colorado. You will work to improve or minimize disabilities and help patients return to their best possible functioning, under PT supervision.

The role is PRN to part-time, with day shifts typically 8am-5pm and some weekends as needed. Compensation ranges from $38–$45 per hour, with a supportive, family-like team and ongoing training and

Qualifications

• Licensed Physical Therapy Assistant required and current state license in good standing.
• One year home health experience preferred.
• Graduate with PTA degree from an accredited program.

• Provide physician prescribed physical therapy in patients' homes.
• Improve or minimize residual physical disabilities of the patient.
• Return the patient to optimum and productive level within their capability.
• Collaborate with other home care staff in patient care planning.

What we’re looking for:

• A passion to serve and help others live their best lives possible.
• Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy Assistant from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
• Currently licensed in the state in which practicing
• One (1) year experience preferred in a home health setting

Schedule:

• Day shift
• Monday to Friday 8am-5pm
• Weekends as needed

License/Certification:

• CPR Certification (Preferred)
• Licensed Physical Therapy Assistant (Required)

Job Type and Pay:

• $38-45/hr
