Job Summary and Qualifications
The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with physicians and the patient care team. The PT completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The PT provides or directs most of the patient’s physical therapy to support patients throughout the continuum of care.
