Memorial Home Services in Jacksonville, IL is seeking a part-time Physical Therapist to provide and coordinate comprehensive patient care. Responsibilities include evaluation, planning and implementing treatment plans in line with established standards and guidelines. You must hold a PT degree and IL license (or license pending with ATT) and CPR certification.

Competenze

• Graduate of an approved school of Physical Therapy.
• Basic knowledge in physical therapy practices in accordance with the P.T. Practice Act.
• CPR certification is required.

• Provide and coordinate comprehensive PT care for patients of all ages through evaluation, planning and treatment implementation.
• Ensure compliance with established standards, policies, procedures, protocols and guidelines.

Part time$37.43 - $58.02As a Physical Therapist, you will be responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines. Bachelor’s degree in Physical Therapy or entry level equivalent is required. Must be Licensed or license pending in the State of Illinois and without previous disciplinary action in Illinois or any other state. Current CPR card.
