Job summary

Community Health Systems, Inc. is seeking a Physical Therapist at Merit Health River Region. The role evaluates, plans, and administers therapies to children and adults as prescribed by a physician, with goal-setting and regular reassessment.

Responsibilities include developing individualized treatment plans, documenting progress, educating patients and families, and collaborating with the interdisciplinary team to ensure quality care in a fast-paced clinic setting.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• 0–2 years of experience in a physical therapy setting required.
• Licensed PT in the state of employment; BLS certification within 30 days.

Responsibilities

• Conducts thorough PT evaluations and develops individualized treatment plans.
• Administers therapy treatments and documents interventions.
• Reassesses and adjusts plans based on patient progress.
• Educates patients and families on exercises and care.
• Collaborates with physicians and health care team to ensure continuity of care.
