Full-Time Physical Therapist Case Manager, Level 1
Territory: Santa Clara County, with multiple small coverage areas.
Your role involves evaluating and addressing patient needs in their homes, helping them live safely and independently. Responsibilities include:
Annual salary ranges from AMD120,000 to AMD135,000 plus a $10,000 quarterly bonus. Promotion to PTII with increased salary and bonus is possible based on performance. Reassignment to support business needs outside primary territory may occur.
