The Supportive Services for Veteran Families (SSVF) Case Manager equips low-income and homeless Veterans with the resources and support needed to achieve long-term housing stability. The position combines trauma-informed case management, rapid rehousing, homeless prevention strategies, and employment navigation to ensure every Veteran has a pathway to permanent housing and a living wage career.
Job Responsibilities
Comprehensive Case Management – Conduct assessments, develop Housing Stability Plans, and provide intensive, strengths-based case management aligned with SSVF guidelines.
Housing Navigation – Identify safe, affordable housing options (including master leased units, shared housing, and shallow subsidy models); and landlord incentives; coordinate move in assistance.
Benefit & Resource Connection – Link Veterans to VA health care, public benefits, behavioral health services, legal aid, and community resources that address barriers to housing and employment.
Financial & Employment Coaching – Deliver budgeting, credit repair, job readiness, and job placement services in partnership with Mission Services Business Developers.
Data & Compliance – Maintain timely, accurate case notes in HMIS/Case Worthy; complete SSVF forms, VISPDATs, and quarterly recertifications; ensure files meet VA audit standards.
Performance Targets – Meet or exceed contract metrics for enrollments, permanent housing placements, housing retention, and client income growth.
Collaboration & Outreach – Build relationships with HUDVASH, County CoC partners, landlords, and veteran-serving agencies; conduct outreach presentations to generate referrals.
Crisis Intervention – Employ de-escalation and harm reduction techniques; respond to urgent Veteran needs during regular business hours and participate in a rotating on call schedule as needed.
Other Duties – Support program innovation, assist with grant reporting, deliver workshops, and perform additional tasks as assigned.
Preferred Qualifications/Experience
Bachelor’s degree in Social Work, Human Services, Psychology, or related field –or– two (2) years of fulltime case management with homeless or Veteran populations.
United States Veteran and/or deep knowledge of military culture and Veterans benefits.
Current CAADE/CADTP/CADAC certification or experience working with cooccurring disorders.
Familiarity with SSVF regulations, HUD homeless programs, Fair Housing law, and trauma-informed care.
Experience using HMIS (Bit focus Clarity), CaseWorthy, or comparable case management systems.
Reliable vehicle, valid California driver’s license, insurance, and an excellent driving record.
Required Qualifications:
Minimum one (1) year professional case management experience.

Working conditions
Indoor, climate controlled office with routine community fieldwork (home visits, outreach events). Regular contact with the public, government agencies, and partner nonprofits. Minimal direct supervision; high degree of independence.

Moderate: Frequent deadlines and crisis situations; fastpaced environment requiring sustained attention and teamwork.
Physical requirements
The job involves sedentary and light work. Light work requires lifting no more than 20 lbs and frequent lifting and or carrying items weighing 10 lbs.
Light work differs from sedentary work in that it requires substantially more walking and standing, and the lifting required is frequent; or that repetitive manipulation of clothing or other items while seated or standing is frequent.
