Pleasant Acres Rehabilitation and Nursing Center is looking for compassionate and skilled PRN Physical Therapist to join our in-house rehab team!

Responsibilities:

• Plan treatment for residents and patients with disabilities to relieve pain, restore or improve function, and promote healing.
• Ensure all treatment is delivered according to the established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Monitor patient responses to treatments.
• Complete necessary documentation in accordance with policies and regulations.
• Assess and interpret patient evaluations and test results.
• Develop treatment plans using various techniques.
• Create personalized health care programs for patients.
• Administer prescribed physical therapy treatments to improve mobility and relieve pain.
• Advise patients on exercise techniques and in-home treatment options.
• Educate about injury prevention, ergonomics, and health promotion.
• Collaborate with other healthcare professionals.
• Document patient care history.
• Comply with rules, regulations, and procedures.
