Job summary

Worland Health and Rehabilitation in Worland, WY is seeking a Physical Therapist to join our fast-paced team. The position focuses on evaluating residents, creating and monitoring treatment plans, and collaborating with physicians and the Rehab Manager to ensure quality patient care.

Responsibilities include administering PT services, documenting encounters, participating in discharge planning, and contributing to continuous quality improvement within the center.

Qualifications

• Bachelor, Master of Science, or Doctorate in Physical Therapy from an accredited program.
• Successful completion of National Certification Exam for PT.
• Current licensure in State of practice where applicable.

• Document treatment encounters in medical records and forms.
• Complete progress notes per Center policy.
• Participate in QA reviews and denials processes as requested.

Job Summary:

Worland Health and Rehabilitation is seeking a Physical Therapist to join our exciting and fast paced team! Our housekeepers provide daily housekeeping services to maintain the Center's safety, sanitation, and attractive condition.

Essential Functions:

• Comply with state practice act and/or American Physical Therapy Association (APTA) guidelines.
• 2. Develop and implement treatment plans that focus on functional and measurable outcomes with physician order. Monitor residents' responses to treatment and alter the treatment plan as appropriate with physician approval.
• 3. Participate in continuous quality improvement activities.
• 4. Identify and advise Rehabilitation Manager (RM) of any Center, staff, or operations issues that need to be addressed.
• 5. Communicate resident/staff issues affecting assigned caseload.
• 6. Develop/implement Physical Therapy services in conjunction with the RM and Center staff.
• 7. Participate in discharge planning.
• 8. Demonstrate supervisory activities in resident charts and service logs as applicable.
• 9. Participate in RAFT (Rehab Advocating Function Today) activities to identify residents in need of rehab services.
• 10. Provide physical therapy screenings and evaluations to determine residents' functional level before initiation of treatment.
• 11. Maintain productivity at level determined by the RM.
• 12. Supervise PT assistants and aides if applicable.
• 13. Self-monitor productivity level daily.
• 14. Participate in resident/family meetings as well as Center meetings when appropriate.
• 15. Follow administrative policies and procedures related to emergency procedures, infection control, and safety.
• 16. Maintain consistent and predictable hours of service in buildings as assigned.
• 17. Provide clear, concise documentation in medical record and on appropriate forms as it relates to reimbursement guidelines and Center policy and procedure.
• 18. Document and sign each treatment encounter in medical record.
• 19. Complete progress notes per Center policy.
• 20. Participate in Quality Assurance Review and denials process as requested by Rehab Manager.
