Job summary

Innovate Rehab and Wellness in Winnfield, LA is seeking a licensed Physical Therapist to join our skilled nursing facility team. The role emphasizes one-on-one patient care and evidence-based treatment plans to restore movement and improve quality of life.

You will evaluate patients, develop personalized plans, and work with physicians, nurses, and other clinicians. Flexible Monday–Friday schedule and full-time benefits are provided.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.

Position Summary
