Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Hilliard, Ohio. You will evaluate, develop, and implement individualized treatment plans to help residents improve mobility, restore function, reduce pain, and maintain independence.

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise PTAs in accordance with state regulations.
• Complete timely and accurate EMR documentation.
• Educate residents and caregivers on treatment plans and home exercise programs.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

About the Role We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.


• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.


• Maintain a productive clinical caseload while delivering exceptional therapy services.
