Job summary

Eagle Rock Post Acute Care at Cedar Grove is seeking a licensed Physical Therapist to join our skilled nursing facility. You will collaborate with the therapy team, perform evaluations, document care, and deliver therapies per prescribed plans to support patient outcomes.

Ideal candidates have NJ licensure, experience in long-term care, and strong organizational and communication skills to manage diverse patient needs while ensuring compliant billing practices.

Qualifications

• Active New Jersey PT license.
• Experience in long-term care (Skilled Nursing Facility).
• Strong organizational, interpersonal and communication skills.
• Ability to manage patients with different personalities and levels of ability.

Responsibilities

• Collaborate with therapy teams and Rehab Department.
• Complete evaluations and documentation.
• Provide therapy per treatment plans and prescriptions.
• Manage workflow and document group and individual treatments in EHR.
• Align therapy plans with Medicaid/Medicare billing considerations.

Local Skilled Nursing Facility Now seeking a Physical Therapist

Responsibilities/duties:

• Work collaboratively with the therapy teams and Rehab Department
• Complete evaluations and all necessary documentation
• Provide recommendations supporting communications/assessments
• Provide therapy as outlined in treatment plans and prescriptions
• Offer treatments that help improve patient's inabilities and/or challenges
• Independently manage workflow which includes allotting time for treatment documentation of individual and group treatments and notes in the electronic records system
• Familiar with PDPM
• Align therapy treatments and plans with understanding of Medicaid and Medicare billing

Qualifications:

• New Jersey License
• Background/experience in long term care (specifically a Skilled Nursing Facility)
• Strong organizational, interpersonal and communication skills
• Ability to manage patients with different types of personalities and ability levels
• Creative problem solving skills
