AdvisaCare in Michigan is seeking a licensed Physical Therapist to join our home health team. You will evaluate patients, design personalized treatment plans, and deliver compassionate, patient-centered care in clients' homes.

The role emphasizes mobility, safety, and independence, with travel between patient locations, collaboration with a multidisciplinary team, and opportunities for growth within AdvisaCare.

Qualifications

• Must hold a valid Physical Therapist license in Michigan.
• Minimum 1 year in home health care preferred.
• Strong communication and interpersonal skills.
• Ability to work autonomously and manage schedules.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility, strength, balance, and function
• Perform therapeutic exercises and interventions to restore independence and reduce pain
• Assess patient progress and adjust plans as needed, coordinating with care teams
• Educate patients and families on exercises, safety, and injury prevention
• Maintain accurate documentation and comply with legal and professional standards

Responsibilities

What We're Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan
• Experience - while a minimum of one year in home health care is preferred, we're open to fresh perspectives and new talent!
