Job summary

Sweetwater Care is expanding its in-house therapy team in Visalia, California. A licensed Physical Therapist will help patients recover from serious illnesses or injuries, enhance mobility, balance, and gait, and support overall well-being within an integrated IDT.

The role emphasizes collaboration with the therapy team, documentation for accurate billing, and adherence to the Plan of Care alongside federal and state guidelines.

Qualifications

• Requires valid Physical Therapy license or eligibility in the state of practice.
• Experience providing rehab and mobility-focused care preferred.
• Ability to document services for billing and care plans.

Responsibilities

• Provide support care for residents to recover and rehabilitate.
• Assess and set goals to return to POLF.
• Document all services for timely billing.
• Follow Plan of Care and professional guidelines.
• Develop and implement individualized therapy programs.
• Identify need for referrals to other disciplines.
• Other duties as assigned.

Position Description

A PT is responsible for helping patients recover and rehabilitate from the effects of a serious illness, injury or medical event, regaining their own best possible level of functional ability, well-being and independence. Provides support services to improve strength and function, restore more efficient movement, improve flexibility and aid patients in restoring mobility and/or improving balance and gait after hip fractures and other orthopedic injuries, medical illness, or as they recover from joint replacement procedures.

Principal Responsibilities

• Provide support care of residents who seek short-term/transitional care with the goal to recover and rehabilitate from the effects of serious illness, injury or medical event, regaining their own best possible level of functional ability, well-being and independence.
• Assess and determine the appropriate aspects of quality patient care and create proper patient goals to return to POLF.
• Document all services provided to ensure accurate and timely billing of therapy services.
• Assures that patient care follows the established Plan of Care, the organization s guidelines, meets professional standards, community needs and is in compliance with Federal and State guidelines.
• Identify and develops a comprehensive plan of care; Develops and implements individualized therapy programs designed to restore, reinforce and enhance performance.
• Identify need for and makes referral to other disciplines to address the comprehensive needs of the patient.
• Other duties, responsibilities and activities may change or assigned at any time with or without notice.
