Job summary

Sweetwater Care and Rehab is seeking an Occupational Therapist to join an in-house therapy team. The role focuses on rehabilitation for cognitive and ADL disorders, with emphasis on objective measures and regulatory compliance.

The position reports to Rehabilitation Services and requires active state licensure, NDOT/AOTA adherence, and collaboration with the IDT to support patient goals and discharge planning.

Qualifications

• Active occupational therapy licensure in California.
• NBCOT / AOTA compliance required.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop an appropriate plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per standards.
• Administer ADL retraining, therapeutic activities, exercises, and assistive devices.
• Assess progress and adjust treatments in the clinical record.
• Communicate patient progress and plans with supervisor and health team.
• Provide discharge summaries per regulatory requirements.
• Document all regulatory and clinical information in the electronic system.
• Supervise and support occupational therapist assistants.
• Participate in required meetings and trainings.

Principal Responsibilities

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function, and, when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

• Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops an appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan, including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, assistive devices, etc.
• Assesses patient’s progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
• Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participation in required meetings, including but not limited to patient care conferences, utilization review meetings, rehabilitation meetings, etc.
• Participate in facility and company-required training, in‑services, and conferences.
• Able to supervise and support occupational therapist assistants
• Adherence to NBCOT /AOTA and State Required practice acts, code of conduct, and ethics during professional practice as an Occupational Therapist.
• Adherence to Company required policy and procedures and code of conduct.
