Job summary

Sweetwater Care is expanding its in-house Therapy Team in the Central Valley. We seek qualified Physical Therapists to help residents recover and regain independence after illness or injury. The role focuses on client-centered care and improving mobility, strength, and balance.

Positions are full-time and PRN, with opportunities to develop a dedicated therapy department and collaborate across disciplines to meet patient needs. California licensure is required, Open to new graduates welcome.

Qualifications

• Requires valid Physical Therapy license in the state of practice.
• Graduated from accredited Physical Therapy program (Open to new graduates)

Responsibilities

• Provide care of residents who seek short-term/transitional care with the goal to recover and rehabilitate from the effects of serious illness, injury or medical event, regaining their own best possible level of functional ability, well being and independence.
• Demonstrate clinical reasoning in synthesizing complex evaluation data identifying diagnosis and developing a comprehensive plan of care.
• Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance.
• Identify need for and make referral to other disciplines to address the comprehensive needs of the patient.
• Other duties, responsibilities and activities may change or assigned at any time with or without notice.
