Job summary

Best Home Health Providers, Inc. is seeking a per diem Physical Therapist in California to deliver skilled home health care, helping patients regain mobility and independence. The role supports patients throughout their recovery journey in a compassionate, family-oriented team.

The position requires a PT degree, active CA license, 2+ years of PT experience (home health a plus), CPR certification, and a valid driver's license with reliable vehicle. Per visit compensation ranges $160–$180.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to create individualized treatment plans.
• Deliver physical therapy services per the physician’s plan of care and California’s PT Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team, reporting changes in patient condition and participating in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties required for patient services.
