Best Home Health Providers, Inc. is seeking a per diem Physical Therapist to deliver skilled home health care across the Bay Area and surrounding regions. You will evaluate patients in their homes, develop individualized plans, and guide recovery to regain mobility and independence.

The role requires a CA PT license, 2+ years of experience, CPR certification, and a valid driver's license. Per-visit compensation ranges from $180 to $200, with flexible scheduling and potential FT conversion based

Qualifications

• CA Physical Therapy license required.
• Minimum 2 years of PT experience; home health a plus.
• CPR certification and driving license required.

Responsabilités

• Evaluate functional status, mobility, strength, gait and ROM to inform treatment plans.
• Deliver PT services per physician's plan of care and CA PT Act.
• Educate patients and families on exercises, equipment use, and goals.
• Collaborate with physicians and care team; report changes and attend case conferences.
• Supervise PT assistants and oversee HHA care when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.

Location: On the road, Patients' Homes

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
• Providing therapy to patients in their home-setting.
