Job summary

Best Home Health Providers Inc. is seeking a per diem Physical Therapist to deliver skilled therapy in patients' homes across the Bay Area and beyond.

You will assess mobility, design plans of care, and collaborate with physicians and family caregivers to support safe recovery. This on-the-road role offers a flexible per diem schedule, mileage reimbursement, and potential to convert to full-time based on performance.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background helpful.
• CPR certification (AHA) is required.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team—report changes and participate in case conferences.
• Supervise PTAs and, when PT is sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.

Please note that this role is per diem with a rate of $180 - $200 per visit.

Responsibilities

• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
• Providing therapy to patients in their home-setting.
