Job summary

Best Home Health Providers Inc. is seeking a Physical Therapist to provide skilled home health services across the Bay Area and beyond. The role is per diem, traveling to patients’ homes with a pay rate of $180–$200 per visit.

You will assess function, develop treatment plans, and educate patients and families to support recovery and independence. The position requires a California PT license, possession of CPR (AHA) and a valid driver’s license, with 2+ years of PT experience; home health

Qualifications

• PT degree from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years PT experience; home health is a plus.
• Strong communication skills with patients and families.
• CPR certification (AHA) required.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to tailor treatment plans.
• Deliver PT services per physician’s plan of care and CA PT Practice Act.
• Educate patients and families on exercises, equipment, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care when PT is solo skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
• Providing therapy to patients in their home-setting.
