Job summary

Best Home Health Providers, Inc. is seeking a Physical Therapist to support patients across the Bay Area on a per-diem basis, delivering skilled home health care and helping patients regain mobility and independence.

The role requires a California Physical Therapy license, a degree in PT, and a minimum of 2 years' PT experience. CPR certification and a reliable vehicle are also needed. Compensation ranges from $160 to $190 per visit, with potential for full-time conversion based on performance.

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans
• Deliver physical therapy services in accordance with the physician's plan of care and California's Physical Therapy Practice Act
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly
• Manage documentation including clinical progress summaries and discharge planning
• Identify needs for additional home health services and coordinate referrals as appropriate
• Perform other duties as required for patient services
