Job summary

Elara Caring seeks a Physical Therapist to join our home-based care team in Tulsa, Oklahoma. You will evaluate patients, develop treatment plans, and provide therapy services in the home setting.

The role emphasizes collaboration with physicians and caregivers, documentation, and supervision of therapy staff as appropriate; requires an APTA-approved program, current Oklahoma PT license, and travel between patient homes.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work (Oklahoma).
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including patient transfers.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
