Job summary

AdvisaCare is seeking a licensed Physical Therapist to provide in-home care across Michigan. The role focuses on evaluating patients, creating personalized treatment plans, and guiding mobility and independence with compassionate, patient-centered approaches.

You will work with a supportive interdisciplinary team, educate families, and maintain accurate records while traveling to patient locations in the Traverse City area. Flexibility and reliability are essential.

Qualifications

• Licensed Physical Therapist with Michigan license required.
• Minimum 1 year home health care experience preferred, open to new talent.
• Demonstrated teamwork and strong communication with patients and families.
• Self-motivated with ability to manage schedules and patient progress.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility and function.
• Perform therapeutic exercises and interventions to regain independence and reduce pain.
• Assess and document patient progress and adjust plans as needed.
• Collaborate with interdisciplinary teams to ensure continuity of care.
• Educate patients, families, and caregivers on exercises and safety.
• Maintain accurate documentation and adhere to professional standards.

• Assess and document patient progress, modifying treatment plans as needed
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards

Requirements

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan
• Experience - while a minimum of one year in home health care is preferred, we're open to fresh perspectives and new talent!
• Team Player - you should have excellent communication skills and the ability to engage with patients and families effectively
• Compassionate Caregiver - a genuine desire to help others and promote their independence is vital
• Self-Starter - the ability to work autonomously, manage your own schedule, and track patient progress is key
