AdvisaCare seeks a licensed Physical Therapist in Michigan to deliver patient-centered home health services. You’ll evaluate patients, design individualized plans, and work within an interdisciplinary team to restore mobility and function.

The role emphasizes compassionate, self-directed care with travel to patient locations, documentation, and ongoing communication with families and caregivers.

Qualifications

• Licensed Physical Therapist in Michigan.
• Minimum of one year in home health care preferred.
• Excellent communication skills with patients and families.
• Self-motivated, autonomous, and able to manage own schedule.
• Valid driver’s license and reliable vehicle for travel.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility and function.
• Perform therapeutic exercises and interventions to reduce pain and enhance independence.
• Assess progress and modify plans as needed.
• Collaborate with interdisciplinary care teams to ensure continuity of care.
• Educate patients, families, and caregivers on exercises and safety.

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
• Assess and document patient progress, modifying treatment plans as needed
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards

Requirements

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan
• Experience - while a minimum of one year in home health care is preferred, we're open to fresh perspectives and new talent!
• Team Player - you should have excellent communication skills and the ability to engage with patients and families effectively
• Compassionate Caregiver - a genuine desire to help others and promote their independence is vital
