AdvisaCare is seeking a Licensed Physical Therapist to provide patient-centered home health care and therapy services. The role involves evaluating patients, setting goals, and delivering personalized treatment plans in community settings.

The position offers part-time and full-time options, with opportunities to travel to patient locations, educate families, and collaborate with an interdisciplinary care team for optimal outcomes.

Qualifications

• Licensed Physical Therapist with active state license (Michigan).
• Preferably 1 year of home health care experience.
• Strong communication and teamwork skills.
• Self-motivated with ability to manage schedule and patient progress.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility, strength, balance, and function.
• Conduct comprehensive evaluations and establish goals of care.
• Educate patients and families on exercises, safety, and injury prevention.
• Collaborate with interdisciplinary teams to ensure continuity of care.
• Document progress and maintain compliance with clinical standards.

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
• Assess and document patient progress, modifying treatment plans as needed
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards

What We’re Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan.
• Experience - while a minimum of one year in home health care is preferred, we’re open to fresh perspectives and new talent!
• Team Player - you should have excellent communication skills and the ability to engage with patients and families effectively.
• Compassionate Caregiver - a genuine desire to help others and promote their independence is vital.
• Self-Starter - the ability to work autonomously, manage your own schedule, and track patient progress is key.
