Job summary

Qualifications

• Must hold Wisconsin licensure as a Physical Therapist.
• Minimum 2 years of clinical experience, preferably in assisted living or skilled nursing.
• Strong communication and interpersonal skills; ability to work independently and in a team.

Responsibilities

• Conduct thorough evaluations of residents to assess physical abilities and limitations.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Perform therapeutic exercises, manual therapy, and modalities as prescribed.
• Monitor progress and adjust treatment plans accordingly.
• Educate residents and families on exercises to improve mobility and function.
• Collaborate with occupational therapists and physicians to coordinate care.
• Maintain accurate and up-to-date patient records.
• Adhere to American Physical Therapy Association and state licensing standards.
• Travel between Lodge at Whispering Pines and Arbor Lodge Wisconsin Rapids as needed.

Job description

Position: Physical Therapist

Salary Range: $39.78 - $42.40 per hour

Schedule: 30 hours per week, Monday-Friday, no weekend rotation

Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical abilities and limitations
• Develop individualized treatment plans based on patient needs and goals
• Implement treatment plans, including therapeutic exercises, manual therapy, and modalities
• Monitor and document patient progress, adjusting treatment plans as needed
• Educate patients and their families on exercises and techniques to improve mobility and function
• Collaborate with other healthcare professionals, such as occupational therapists and physicians, to coordinate patient care
• Maintain accurate and up-to-date patient records
• Adhere to all professional and ethical standards set by the American Physical Therapy Association and state licensing board
• Travel between The Lodge at Whispering Pines and Arbor Lodge Wisconsin Rapids as needed to provide services to residents at both locations

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Current state license to practice as a Physical Therapist
• Minimum of 2 years of experience in a clinical setting, preferably in an assisted living or skilled nursing facility
• Strong knowledge of physical therapy principles and techniques
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Willingness to travel between locations as needed
