Job summary

Faith Home Health & Hospice is seeking a licensed Physical Therapist to provide in-home therapy in Topeka, KS, and surrounding communities. You will collaborate with physicians and an interdisciplinary team to maximize patient independence, safety, and quality of life.

Qualifications

• Current Kansas Physical Therapist License required.
• Graduate of an accredited Physical Therapy program (APTA or AMA approved).
• Home Health experience preferred.
• Minimum of two years of clinical experience preferred.
• Current CPR certification required.

Responsibilities

• Complete initial patient evaluations within 48 hours of referral and re-evaluations every 30 days.
• Develop, implement, and modify individualized treatment plans in collaboration with physicians and team members.
• Maintain timely clinical documentation including evaluation reports, daily notes, progress updates, and discharge summaries.
• Provide education to patients and families on safe mobility techniques, equipment, fall prevention, and home exercise programs.
• Communicate effectively with patients, caregivers, physicians, and agency staff for coordinated, high-quality care.
• Participate in case conferences, chart audits, and quality improvement initiatives.

• Participate in case conferences, chart audits, and quality improvement initiatives to ensure regulatory compliance and positive patient outcomes.

Qualifications

• Current Kansas Physical Therapist License required.
• Graduate of an accredited Physical Therapy program (APTA or AMA approved).
• Home Health experience preferred.
• Minimum of two years of clinical experience preferred.
• Current CPR certification required.
• Valid driver's license, current auto insurance, and reliable transportation.
• Demonstrated ability to work independently while maintaining strong communication with the care team.
