Job Summary
As a Physical Therapist, you will provide skilled in-home therapy services based on individualized treatment plans, working collaboratively with physicians and interdisciplinary team members to maximize patient independence, safety, and quality of life.
What You'll Do
- Complete initial patient evaluations within 48 hours of referral and conduct re-evaluations every 30 days to ensure appropriate plan of care.
- Develop, implement, and modify individualized treatment plans in collaboration with physicians and interdisciplinary team members.
- Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries.
- Provide education to patients and families regarding safe mobility techniques, adaptive equipment, fall prevention, and home exercise programs.
- Communicate effectively with patients, caregivers, physicians, and agency staff to support coordinated, high-quality care.
- Participate in case conferences, chart audits, and quality improvement initiatives to ensure regulatory compliance and positive patient outcomes.
Qualifications
- Current Kansas Physical Therapist License required.
- Graduate of an accredited Physical Therapy program (APTA or AMA approved).
- Home Health experience preferred.
- Minimum of two years of clinical experience preferred.
- Current CPR certification required.
- Valid driver's license, current auto insurance, and reliable transportation.
- Demonstrated ability to work independently while maintaining strong communication with the care team.
