THERAPY 2000 is a mission-driven team providing pediatric Speech, Occupational, and Physical Therapy in Texas families' homes. The Physical Therapist will evaluate children, design treatment plans, and collaborate with caregivers and medical teams to support each child’s growth in a comfortable home setting.

We seek an innovative, reliable professional with an active Texas PT license who can adapt to changing needs, deliver high-quality care, and participate in ongoing education opportunities

Qualifications

• Active Texas Physical Therapy license required.
• Ability to work autonomously in home-care settings.
• Strong communication and coordination with care teams.

Responsabilités

• Perform comprehensive Pediatric PT evaluations using standardized tests.
• Develop and implement treatment plans per physician orders.
• Create home exercise programs with caregivers.
• Collaborate with other providers to coordinate care.
• Complete required patient visits weekly and maintain charts.
• Meet deadlines for progress notes, call logs, and documentation.
• Ensure all duties comply with home-health policies and laws.

Qualifications

What you’ll need:

• A current/active license to practice Physical Therapy in the state of Texas.
• An innovative spirit complemented by a demonstrated history of reliability.
• Flexibility to shift resources and priorities as necessary, based on the needs of the business.
• Ability to use common workplace technology (laptop computer, phone, email, web browser).

Bonus points for:

• 3+ years as a pediatric Physical Therapist
• Experience in a home-health setting
• Specialty Certifications (example: PCS)
• Bilingual (English/Spanish)

Responsibilities

What you’ll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
