Sunnyvale Gardens Post-Acute is hiring a dedicated Physical Therapist (PT) for a full-time role in Sunnyvale, CA. You will evaluate residents, plan individualized therapy programs, and collaborate with doctors, nurses, and other therapists to promote mobility and independence.

The position requires CA licensure, a PT degree, CPR/BLS, and strong communication skills; new grads are welcome. Competitive hourly pay and comprehensive benefits accompany this role.

• Master’s or Doctorate degree in Physical Therapy from an accredited program.
• Active, unrestricted Physical Therapist license in the State of California.
• Current CPR/BLS certification.
• Experience in a SNF/post-acute or outpatient rehab is preferred, new grads welcome.

Responsabilidades

• Conduct comprehensive initial evaluations to determine rehab potential and mobility.
• Develop, implement, and modify plans of care based on patient needs and physician orders.
• Provide evidence-based PT treatments including strength, balance, and mobility training.
• Document resident progress and discharge plans per federal/state guidelines.
• Educate residents and families on post-discharge exercises and safety protocols.
• Collaborate with interdisciplinary care team to optimize outcomes.

In this role, you will evaluate, plan, and administer personalized physical therapy programs to help our residents achieve their maximum functional mobility.
