Job summary

Sunnyvale Gardens Post-Acute in Sunnyvale, CA seeks a compassionate Physical Therapist to join our full-time rehabilitation team. You will evaluate, plan, and implement personalized therapy programs to help residents regain mobility and independence.

You will work with doctors, nurses, and fellow therapists, document progress, educate residents and families on post-discharge exercises, safety, and devices, and contribute to a patient-centered, collaborative care environment.

Qualifications

• Master’s or Doctorate in Physical Therapy from an accredited program.
• Active, unrestricted California Physical Therapist license.
• Current CPR/BLS certification.
• Experience in a skilled nursing facility or post-acute rehab preferred; new grads welcome.

Responsibilities

• Conduct comprehensive initial evaluations to assess physical condition and rehab potential.
• Develop, implement, and modify individualized plans of care based on resident needs and physician orders.
• Provide evidence-based PT treatments (strength, balance, mobility) and document progress.
• Educate residents and families on post-discharge exercises, safety, and assistive devices.
• Collaborate with doctors, nurses, and therapists to optimize patient outcomes.

In this role, you will evaluate, plan, and administer personalized physical therapy programs to help our residents achieve their maximum functional mobility. You will work closely with a multidisciplinary team of doctors, nurses, and fellow therapists to deliver top-tier post-acute care.Key ResponsibilitiesConduct comprehensive initial evaluations of residents to determine their physical condition, limitations, and rehabilitation potential.Develop, implement, and modify individualized plans of care based on resident needs and physician orders.Provide high-quality evidence-based physical therapy treatments (including strength training, balance exercises, and mobility training).Accurately document resident progress, treatments, and discharge plans in compliance with federal, state, and facility guidelines.Educate residents and their families on post-discharge exercises, safety protocols, and assistive devices.Collaborate effectively with the interdisciplinary care team to optimize patient outcomes.Qualifications & RequirementsEducation: Master’s or Doctorate degree in Physical Therapy from an accredited program.Licensure: Active, unrestricted Physical Therapist license in the State of California.Certification: Current CPR/BLS certification.Experience: Experience in a skilled nursing facility (SNF), post-acute care, or outpatient rehab setting is preferred, but passionate new graduates are welcome to apply!Strong communication, interpersonal, and time-management skills.What We Offer (Benefits & Perks)We value our therapists and believe in taking care of the people who take care of our residents.
