Role Description

This is a contract, on-site Physical Therapist role based in Sebring, FL. The Physical Therapist will assess patients’ functional status and mobility, develop individualized treatment plans, and provide evidence-based therapeutic interventions in the home health setting. Daily responsibilities include performing comprehensive evaluations, implementing and progressing rehabilitation programs, educating patients and caregivers on exercises and safety, and documenting clinical findings and outcomes in accordance with regulatory standards. The role also involves collaborating with physicians, nurses, and other care team members to coordinate services, monitor progress, and adjust care plans as needed. The Physical Therapist will ensure adherence to best practices, infection control, and home safety guidelines while supporting patients in achieving their rehabilitative goals.

Qualifications

• Demonstrated expertise in Physical Therapy and Rehabilitation, including evaluation, treatment planning, and therapeutic interventions.
• Strong background in Medicine and Patient Care, with the ability to collaborate effectively within an interdisciplinary clinical team.
• Experience in Home Care delivery, including providing therapy in patients’ residences and ensuring safe, patient-centered services.
• Active license as a Physical Therapist in the state of Florida (or eligibility for licensure) and graduation from an accredited Physical Therapy program.
• Solid clinical documentation skills, familiarity with home health regulations, and competency with electronic health records.
• Effective communication, time management, and organizational skills, with the ability to work independently in a community-based setting.
• Current CPR certification and valid driver’s license with reliable transportation to visit patients in their homes.
• Prior home health or geriatric experience is preferred but not required; willingness to travel locally within Sebring, FL and surrounding areas.
