Qualifications

• Active Florida Physical Therapist license or eligible to licensure.
• Experience in evaluation, treatment planning and therapeutic interventions.
• Experience delivering therapy in patients' homes and community settings.
• Strong communication, time management, and organizational skills.
• Ability to travel within Sebring, FL and surrounding areas.

Responsibilities

• Assess patients’ functional status and mobility to develop individualized treatment plans.
• Implement and progress rehabilitation programs and monitor patient progress.
• Educate patients and caregivers on exercises and safety.
• Document clinical findings and outcomes per regulatory standards.
• Collaborate with physicians, nurses, and the care team to coordinate services.
