Job summary

Santa Maria Post Acute is seeking a PRN Physical Therapist to join the In-House Therapy Team for 15 hours per week in Santa Maria, CA. You will plan and deliver PT services, assess patients, and collaborate with a supportive team to promote recovery and functional improvement.

The role involves counseling patients and families, documenting care, and adhering to state regulations. A flexible schedule and in-house training opportunities are available.

Responsibilities

• Plan treatment to residents and patients with disabilities to relieve pain, restore or improve function.
• Assure all treatment is delivered in accordance with an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Monitor patient response to treatment intervention.
• Complete required forms and documentation in accordance with policy and regulations.
• Consult with patients to learn about their physical condition.
• Develop treatment plans using various techniques.
• Create personalized fitness-oriented health care programs for patients.
• Administer physical therapy treatments to relieve pain and improve mobility.
• Advise patients on exercise techniques and home treatment options.
• Provide education on injury prevention and ergonomics.
• Document patient care history and comply with guidelines.

Essential Duties and Responsibilities:

• Plan treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing

• Assure all treatment is delivered in accordance with an established plan of care

• Provide clinical support and instruct patients, families, and caregivers

• Monitor patient response to treatment intervention

• Complete required forms and documentation in accordance with company policy and state/federal regulations

• Consulting with patients to learn about their physical condition.

• Assessing and interpreting patient evaluations and test results.

• Developing treatment plans using a variety of treatment techniques.

• Creating personalized fitness-oriented health care programs for patients.

• Administering medically prescribed physical therapy treatments to relieve pain and improve mobility.

• Advising patients on exercise techniques.

• Advising patients and their families about in-home treatment options.

• Providing educational information about injury prevention, ergonomics and ways to promote physical health.

• Consulting and collaborating with other healthcare professionals.

• Documenting patient care history.

• Complying with rules, regulations, and procedures.
