Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to join our growing team. This role delivers high-quality, individualized care in the comfort of patients' homes while enjoying a flexible schedule and supportive team environment.

As a Field Physical Therapist, you will travel to patient homes, perform SOC/ROC/Recert/DC evaluations, collaborate with the interdisciplinary team, and educate patients and caregivers on home exercise programs.

Qualifications

• Active California Physical Therapy license/registration.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Provide comprehensive skilled physical therapy services and interventions per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations and discharges.
• Assess patient needs, plan interventions, evaluate outcomes, and adjust care to meet goals.
• Collaborate with the interdisciplinary team to set and evaluate patient goals.
• Educate and train patients and caregivers on PT programs and home exercises.

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
