Job summary

Roscoe Rehabilitation & Nursing Center in Roscoe, NY seeks a Physical Therapist to evaluate residents, develop individualized plans, and provide skilled therapy to improve mobility and independence.

The role offers a salaried, full-time schedule with shift 8 am–4 pm, a sign-on bonus up to $7,500, and a clear path to growth within our rehabilitation team located in the Catskill Mountains. NY license required; prior SNF experience preferred.

Qualifications

  • Master's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association.
  • Valid New York State Physical Therapist License required.
  • Newly licensed Physical Therapists encouraged to apply.
  • Skilled nursing facility experience preferred.
  • Strong clinical, documentation, communication, and problem-solving skills.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
  • Commitment to providing compassionate, resident-centered care.

Responsibilities

  • Evaluate residents and develop individualized physical therapy treatment plans
  • Establish short- and long-term rehabilitation goals and monitor resident progress
  • Provide skilled therapy services to improve mobility, strength, balance, endurance, and functional independence
  • Supervise Physical Therapist Assistants (PTAs) in accordance with New York State regulations
  • Complete required documentation, evaluations, care plans, and billing records accurately and timely
  • Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes
  • Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs
  • Recommend and coordinate durable medical equipment (DME) and discharge planning needs
  • Participate in care conferences, staff education, orientation, and quality improvement initiatives
  • Maintain therapy equipment, supplies, and treatment areas
  • Perform additional duties as assigned
