Taconic Rehabilitation & Nursing at Beacon in Beacon, NY is hiring a Physical Therapist (PT) to evaluate residents, develop individualized treatment plans, and provide skilled therapy to improve mobility and function. The role requires a NY PT license and offers full-time daytime hours.

Responsibilities include supervising PTAs, documenting care plans, collaborating with physicians and families, and coordinating DME and discharge planning. Prior experience in skilled nursing is preferred.

Qualifications

• Master's or Doctorate Degree in Physical Therapy from an accredited program
• New York State Physical Therapist License required
• Strong clinical, documentation, communication, and problem-solving skills
• Ability to work collaboratively within an interdisciplinary healthcare team

Responsabilités

• Evaluate residents and develop individualized physical therapy treatment plans
• Establish short- and long-term rehabilitation goals and monitor progress
• Provide skilled therapy services to improve mobility, strength, balance, endurance, and functional independence
• Supervise PTAs in accordance with New York State regulations
• Complete required documentation, care plans, and billing records accurately and timely
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes
• Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs
• Coordinate DME and discharge planning needs
• Participate in care conferences, staff education, orientation, and quality improvement initiatives
• Maintain therapy equipment, supplies, and treatment areas

RESPONSIBILITIES:

• Evaluate residents and develop individualized physical therapy treatment plans

• Supervise Physical Therapist Assistants (PTAs) in accordance with New York State regulations
• Complete required documentation, evaluations, care plans, and billing records accurately and timely
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes
• Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs
• Recommend and coordinate durable medical equipment (DME) and discharge planning needs
• Participate in care conferences, staff education, orientation, and quality improvement initiatives
• Maintain therapy equipment, supplies, and treatment areas
• Perform additional duties as assigned

REQUIREMENTS:

• Master's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association
• Valid New York State Physical Therapist License required
• Newly licensed Physical Therapists encouraged to apply
• Skilled nursing facility experience preferred
• Strong clinical, documentation, communication, and problem-solving skills
• Ability to work collaboratively within an interdisciplinary healthcare team
