Job summary

Roscoe Rehabilitation & Nursing Center is seeking a Physical Therapist Assistant to join the rehabilitation team. You will provide skilled PT services under supervision to help residents improve mobility, strength, balance, and overall function.

The role offers a rate of $30-$33 per hour, with a 8 am–4 pm schedule, and both full-time and part-time options. Newly licensed PTAs are encouraged to apply and experience in skilled nursing is preferred.

Qualifications

  • Associate degree from an accredited PTA program.
  • New York State PTA license required.
  • Newly licensed PTAs encouraged to apply.
  • Skilled nursing facility experience preferred.
  • Strong communication, documentation, and organizational skills.
  • Ability to work collaboratively within an interdisciplinary healthcare team.

Responsibilities

  • Provide skilled physical therapy treatments under the direction of a Physical Therapist
  • Assist with implementing individualized treatment plans designed to improve mobility, strength, balance, and functional independence
  • Monitor resident progress and communicate treatment updates to the supervising Physical Therapist
  • Complete required documentation, treatment records, and care plan updates accurately and timely
  • Educate residents, families, and staff on therapy techniques, exercises, mobility, and adaptive equipment
  • Assist with discharge planning, home exercise programs, and equipment recommendations
  • Collaborate with physicians, nursing staff, and interdisciplinary team members to support resident outcomes
  • Participate in staff training, in-services, and quality improvement initiatives
  • Maintain therapy equipment, supplies, and treatment areas
  • Perform additional duties as assigned
