Job summary

WelbeHealth is seeking a Physical Therapist to join the interdisciplinary IDT and deliver restorative and rehabilitative PT services to participants, aiming to maximize independence and safety.

You will conduct assessments, participate in IDT meetings, deliver interventions, supervise therapy assistants, and train staff and families on equipment care. Compensation includes base pay plus bonus, with a 401(k) match and comprehensive healthcare benefits.

Qualifications

• Bachelor’s degree in physical therapy is required.
• Master’s or doctoral degree is preferred.
• Valid California Physical Therapy License, or proof of license eligibility; CPR and First Aid certification within 180 days of hire.

Responsibilities

• Conduct PT assessments in facility and home visits to define care needs and goals.
• Participate in IDT meetings to develop care plans.
• Deliver and document interventions as per care plans.
• Provide active and passive exercises, gait training, ADL training, and modalities such as massage or electrical modalities.
• Supervise Therapy Assistants and train staff/families on equipment use and care of interventions.
• Assist in maintaining participant safety and independence.

Job description

At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

On the day-to-day, you will:

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Job requirements include:

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
