Role Description

The Physical Therapist is a full-time, hybrid role based in Rhode Island, United States, with some work-from-home flexibility for documentation and care coordination tasks. Day-to-day responsibilities include assessing patients’ functional status, developing individualized treatment plans, and providing therapeutic interventions to improve mobility, strength, and pain management in home and community settings. The Physical Therapist will perform regular patient evaluations, adjust care plans as needed, and collaborate with physicians, nurses, and other allied health professionals to ensure safe, effective rehabilitation. Additional duties include educating patients and caregivers on exercise programs, safety, and equipment use, maintaining accurate clinical documentation, and participating in quality improvement initiatives to advance patient outcomes.

Qualifications

• Demonstrated proficiency in Physical Therapy and Rehabilitation, with the ability to design and implement evidence-based treatment plans.
• Strong skills in Patient Care and Home Care, including safe delivery of services in varied home environments and clear patient/caregiver education.
• Foundational understanding of Medicine related to musculoskeletal, neurological, and cardiopulmonary conditions relevant to physical therapy practice.
• Current license or eligibility for licensure as a Physical Therapist in Rhode Island.
• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• Experience in home health or community-based rehabilitation preferred.Effective communication, time management, and documentation skills, with comfort using electronic health record systems.
• Ability to work independently, travel to patient homes as needed, and uphold inclusive, non-discriminatory care practices.
