Job summary

Innovive Health is seeking a compassionate Physical Therapist to join our growing home health team in Massachusetts. You will provide in-home PT services, create personalized treatment plans, and support patients toward independence, comfort, and safety in their own homes.

You will work with an interdisciplinary team, perform OASIS and documentation, and guide patients through therapeutic exercises and mobility improvements while maintaining strong patient relationships.

Qualifications

  • Licensed Physical Therapist in Massachusetts.
  • Degree from an accredited Physical Therapy program.
  • CPR certification (current).
  • Valid driver's license and reliable transportation.
  • Home health experience required.
  • Strong communication, organization, and clinical decision-making skills.

Responsibilities

  • Provide in-home physical therapy services, including evaluations, start of care visits, follow-ups, recertifications, OASIS assessments, and discharges.
  • Develop and implement personalized treatment plans based on patient needs and physician orders.
  • Guide patients through therapeutic exercises, gait training, balance and coordination activities, and mobility improvement.
  • Educate patients and caregivers on safety, fall prevention, and daily living strategies.
  • Monitor patient progress and adjust treatment plans to achieve optimal outcomes.
  • Assess vital signs as needed and communicate concerns to the care team.
  • Recommend and educate on the use of assistive devices and DME.
  • Collaborate with an interdisciplinary team including nurses, OT, speech therapists, and physicians.
  • Complete timely and accurate documentation.
  • Support discharge planning and ensure continuity of care.

What We're Looking For
  • Licensed Physical Therapist in the state of Massachusetts
  • Degree from an accredited Physical Therapy program
  • CPR certification (current)
  • Valid driver's license and reliable transportation
  • Home health experience required
  • Strong communication, organization, and clinical decision-making skills
Preferred
  • At least 1 year of clinical experience

Work Environment & Physical Requirements
