Job summary

Adoration Health in Providence, RI is seeking a Home Health Physical Therapist to provide 1:1 patient care in a home setting. You will evaluate, treat, and develop plans to help patients reach maximum independence, while coordinating with families and the care team.

The role offers flexible scheduling, independence with team support, and opportunities for growth within a growing company. Licensure in RI and CPR are required.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; community/hospice experience preferred.
• Valid license in the state of practice.
• Maintain current CPR certification.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treating team, physician, patient, and family.
• Maintain thorough documentation for accurate billing.
• Supervise PTAs and PT aides per state act.

Education

Bachelor's Degree in Physical Therapy

New graduates may be considered in select markets

2+ years of physical therapy experience

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification

Salary Range

USD $57.00 / Hour
