Role Description

The Physical Therapist will provide direct patient care in a full‑time, on‑site role based in Plainfield, IL. Day‑to‑day responsibilities include performing comprehensive evaluations, developing and implementing individualized treatment plans, and delivering manual therapy and therapeutic exercises. The role involves educating patients and caregivers on home exercise programs, injury prevention, and functional techniques to support recovery and long‑term health. The Physical Therapist will document clinical findings and progress notes in accordance with professional and regulatory standards, coordinate care with other providers as needed, and contribute to a positive, collaborative clinic culture.

Qualifications

• Strong foundation in Physical Therapy, including knowledge of musculoskeletal and neuromuscular conditions.
• Experience in Rehabilitation, with the ability to design and adjust treatment plans based on patient progress and goals.
• Demonstrated commitment to high‑quality Patient Care, including clear communication, empathy, and professionalism.
• Familiarity with developing safe and effective home exercise programs and self‑management strategies.
• Current state licensure (or eligibility) as a Physical Therapist in Illinois.
• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• Strong documentation and time‑management skills, with attention to clinical detail and regulatory requirements.
• Ability to work collaboratively in a multi‑disciplinary team and to adapt to a dynamic clinical environment.
