Role Description The Physical Therapist will provide direct patient care in an on-site clinical setting located in Dixon, IL. This full-time role includes evaluating patients, developing individualized treatment plans, and delivering evidence-based interventions to support recovery, pain management, and improved function. Day-to-day responsibilities include performing clinical assessments, leading therapeutic exercises, manual therapy, and functional training, as well as educating patients and families on home exercise programs and injury prevention. The Physical Therapist will document care accurately, collaborate with referring providers and the interdisciplinary team, and participate in case conferences and ongoing quality improvement initiatives. The role also involves supporting a positive clinic environment, mentoring support staff, and contributing to patient engagement and satisfaction.

Qualifications

• Candidates should possess strong skills in Physical Therapy and Rehabilitation to assess, plan, and implement effective treatment programs.
• Candidates should possess skills in Medicine and Patient Care to ensure safe, evidence-based clinical decision-making and compassionate interaction with patients.
• Active state licensure as a Physical Therapist (or eligibility to obtain licensure in Illinois) and graduation from an accredited Physical Therapy program (DPT preferred).
• Strong interpersonal, communication, and documentation skills, with the ability to work collaboratively in a multidisciplinary environment.
