Job summary

Deer Meadows Home Health is seeking a Full-Time Physical Therapist (PT) to join our home health team serving Northwest Philadelphia and Montgomery County, PA.

This role offers independence to assess and treat patients in their homes, collaborate with an interdisciplinary team, and help patients regain mobility, balance, and independence while educating families and caregivers.

Qualifications

• Current Pennsylvania Physical Therapist license.
• Home health experience preferred.
• Experience with Homecare Homebase preferred.

Responsibilities

• Provide skilled physical therapy services to patients in their homes.
• Complete evaluations and develop individualized plans of care.
• Provide therapeutic interventions focused on mobility, strength, balance, safety, and functional independence.
• Educate patients, caregivers, and families.
• Coordinate care with physicians, nursing, therapy, and other members of the home health team.
• Complete timely and accurate documentation using Homecare Homebase.

Deer Meadows Home Health is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our home health team!

This is a great opportunity for a PT who enjoys working independently, building meaningful relationships with patients, and providing quality therapy in the comfort of patients’ homes.

Qualifications
