Job summary

New Concepts for Living (NCFL) is seeking a Physical Therapist to provide habilitative and rehabilitative services for adults with intellectual and developmental disabilities in Paramus, NJ. You will lead with clinical expertise and coordinate care across teams to improve functional outcomes.

You will administer assessments, create individualized plans, supervise PTAs, and ensure timely documentation while collaborating with Support Coordinators and healthcare providers to promote coordinated

Qualifications

  • Master’s degree in Physical Therapy required.
  • Current New Jersey Physical Therapy license required.
  • Minimum of 2 years of clinical physical therapy experience.
  • Experience with individuals with developmental disabilities preferred.

Responsibilities

  • Provide habilitative and rehabilitative PT services to adults with I/DD and related needs.
  • Administer and interpret standardized assessments to evaluate function.
  • Develop and update individualized treatment plans aligned with ISP.
  • Supervise and provide clinical direction to Physical Therapist Assistants (PTA).
  • Complete recertifications, progress notes, discharge summaries, and documentation on time.
  • Collaborate with Support Coordinators to implement therapy recommendations in home/community settings.
  • Educate direct care staff on exercises and strategies to be used outside therapy.
  • Participate in multidisciplinary team meetings and contribute clinical expertise.

Physical Therapist (PT)
As a Physical Therapist at NCFL, you’ll:
  • Provide habilitative and rehabilitative physical therapy services to adults with I/DD and individuals with physical, cognitive, and/or psychological needs.
  • Administer and interpret standardized assessments to evaluate functional abilities and identify areas for improvement.
  • Develop individualized treatment plans aligned with each individual's Individualized Service Plan (ISP).
  • Adapt and modify treatment plans and interventions as clinically indicated.
  • Supervise and provide clinical direction to Physical Therapist Assistant(s) (PTA).
  • Complete recertifications, progress notes, discharge summaries, and other required documentation accurately and on time.
  • Communicate with Support Coordinators to promote implementation of therapy recommendations in home and community settings.
  • Educate direct support staff on recommendations, exercises, and strategies to be implemented outside of therapy sessions.
  • Collaborate with healthcare providers and members of the individual's care team to promote effective, coordinated care.
  • Participate in multidisciplinary team meetings and contribute clinical expertise to support positive outcomes.
  • Maintain accurate records of evaluations, progress, treatment plans, and other clinical documentation in compliance with legal, ethical, and agency standards.
  • Follow directives from the Therapy Director and adhere to NJ DDD guidelines for habilitative and rehabilitative therapy services.

Your Schedule:
  • Full-time
  • 45 hours per week
  • Based in Paramus & Rochelle Park, NJ
What You’ll Bring:
  • Master’s degree or higher in Physical Therapy
  • Current New Jersey Physical Therapy license
  • Minimum of 2 years of clinical physical therapy experience
  • Experience working with individuals with developmental disabilities strongly preferred
  • Strong understanding of the unique needs and challenges of individuals with intellectual and developmental disabilities
  • Excellent communication, leadership, and organizational skills
  • Ability to develop and implement individualized treatment plans and clinical initiatives
  • Ability to effectively supervise and provide direction to Physical Therapist Assistant(s)
  • Experience withFoothold AWARDS preferred
